Occupational Therapist (OT) – Madison, TN

 

A rehabilitation healthcare provider is seeking a compassionate and skilled Occupational Therapist (OT) to join its patient care team. This role focuses on helping individuals improve daily living skills, regain independence, and achieve their highest level of functional performance through personalized therapy programs.

 

Responsibilities

  • Evaluate patients to determine functional abilities, limitations, and rehabilitation needs

  • Develop and implement individualized treatment plans based on patient goals and clinical assessments

  • Provide therapeutic interventions to improve mobility, self-care skills, and overall independence

  • Monitor patient progress and adjust treatment plans as needed to support recovery

  • Establish realistic and measurable rehabilitation goals

  • Document evaluations, treatment sessions, progress notes, recertifications, and discharge summaries accurately and timely

  • Educate patients and family members on therapy techniques, adaptive equipment, and home exercise programs

  • Collaborate with physicians, therapists, nursing staff, and other healthcare professionals to coordinate patient care

  • Supervise Occupational Therapy Assistants in accordance with applicable regulations

  • Maintain compliance with patient confidentiality standards and healthcare regulations

  • Support a positive, patient-centered care environment

 

Qualifications

  • Graduate of an accredited Occupational Therapy program

  • Current Occupational Therapist license or eligibility for licensure in the state of practice

  • Strong clinical assessment, treatment planning, and documentation skills

  • Ability to work collaboratively within a multidisciplinary healthcare team

  • Excellent communication and interpersonal skills

  • Experience in rehabilitation, skilled nursing, long-term care, or geriatric settings preferred
