Occupational Therapist (OT) – Travel Contract
Location: Sacramento, CA (Not the right fit? We have additional OT opportunities available throughout California.)
Pay: $2,400-2,600 per week
Contract Length: 13 weeks (extension possible)
Schedule: Day shifts | 36–40 hours per week

We’re looking for a motivated and compassionate Occupational Therapist (OT) to join our Skilled Nursing Facility in Sacramento, CA. Responsibilities

  • Assess and treat SNF residents to improve functional independence.

  • Develop individualized OT treatment plans aligned with care goals.

  • Document therapy progress and adjust plans based on measurable outcomes.

  • Monitor and report productivity and outcome metrics for quarterly targets.

  • Collaborate with interdisciplinary team to coordinate patient care.

  • Utilize evidence-based practices to drive improved patient outcomes.

  • Maintain compliance with regulatory standards and facility policies.

  • Educate residents and families on home programs and safety.

  • Identify and address potential barriers to recovery and participation.

  • Engage in ongoing professional development to advance OT skills.

  • Lead or participate in care conferences to drive patient-centered planning.

  • Manage time efficiently to ensure on-site responsibilities meet targets.


Experience/Qualifications

  • Current OT license in the state of employment with SNF experience.

  • Experience in skilled nursing: assessments, treatment planning, and progress documentation.

  • Ability to develop and adjust individualized OT treatment plans with measurable outcomes.

  • Proficiency in documenting progress and productivity metrics for quarterly targets.

  • Strong collaboration with interdisciplinary team and participation in care conferences.

  • Knowledge of regulatory standards and facility policies; compliance mindset.

  • Evidence-based practice adoption and ongoing professional development.

  • Competence in educating residents and families on home programs and safety.

  • Effective time management to balance on-site duties and targets.

  • Contract role is on-site; willingness to work in growth-focused, merit-based environment.
