Job Summary
Nexus is seeking a compassionate and dedicated Occupational Therapist (OT) for a Float Position supporting our rehabilitation teams across the Wood River, Godfrey, and Alton areas.
This is an excellent opportunity for an OT who enjoys variety, collaboration, and making a meaningful difference across multiple skilled nursing and rehabilitation settings. The Float Occupational Therapist will provide high-quality, patient-centered occupational therapy services based on the needs of each facility.
The ideal candidate is flexible, dependable, clinically confident, and comfortable working with residents across different rehabilitation environments.
Responsibilities
- Evaluate residents and develop individualized occupational therapy treatment plans.
- Provide skilled occupational therapy services in accordance with physician orders and established treatment plans.
- Address deficits related to activities of daily living, functional mobility, strength, coordination, balance, cognition, and upper-extremity function.
- Provide therapeutic activities and interventions designed to improve residents' independence and safety.
- Assist residents with dressing, grooming, bathing, toileting, feeding, transfers, and other activities of daily living as appropriate.
- Provide adaptive equipment recommendations and training when clinically indicated.
- Educate residents, families, and caregivers regarding safety, functional strategies, home programs, and adaptive techniques.
- Monitor resident progress and modify treatment plans as clinically appropriate.
- Complete accurate and timely documentation of evaluations, treatments, progress, and resident responses.
- Communicate resident progress and changes in condition with the interdisciplinary care team.
- Collaborate with physical therapists, speech language pathologists, nursing staff, physicians, and other healthcare professionals.
- Participate in care plan meetings and discharge planning as appropriate.
- Maintain compliance with federal, state, facility, and professional standards.
- Adapt effectively to the needs, workflow, and census of each assigned facility.
- Provide compassionate, respectful, and resident-centered care across all locations.
Qualifications
- Bachelor’s, Master’s, or Doctoral degree in Occupational Therapy from an accredited program, as applicable.
- Current Illinois Occupational Therapist license required.
- Skilled nursing, long-term care, subacute rehabilitation, or geriatric experience preferred.
- Strong clinical assessment and treatment skills.
- Excellent communication and interpersonal skills.
- Ability to work independently and effectively within multiple interdisciplinary teams.
- Flexible and dependable with the ability to travel between assigned locations.
- Compassionate, professional, and resident-focused approach to care.
