Functions in the Patient Admissions Department.

    Job Description

    In collaboration with the oncology nurse navigator, the oncology patient coordinator provides comprehensive support to the coordination of care for oncology patients.  This role serves as a primary point of contact for coordinating new and existing patient referrals. In addition to clerical support, this role will perform registration functions, provide excellent patient focused customer service, and communicate effectively to service delivery areas to maximize patient flow.

    30% - Job Responsibility:  Assist the patient to better direct their own health care experience using screening and reinforcement of the treatment plan, educational programs, and linkage to health services and community resources.
     
    40% Job Responsibility:  Serve as a patient liaison to assist patients and caregivers in scheduling within the complex healthcare system.
     
    30% - Job Responsibility:  Act as an advocate for an individual's health care needs by identifying and communicating opportunities for care intervention that have been established by the RN Navigator or Coordinator, including identifying and addressing gaps in care.

    Additional Job Description

    Education: Bachelor degree from an accredited university and 1 year of healthcare experience or high school diploma and 5 years of job experience in referral management, oncology navigation or patient access. In addition, the applicant must have considerable knowledge of complex medical terminology, strong interpersonal skills, ability to establish and maintain collaborative relationships both internally and externally, strong organizational skills, ability to rapidly prioritize/reprioritize, strong verbal and written communication skills, ability to work autonomously and in teams, critical-thinking skills, and ability to multi-task.

    Experience: 1 year of customer service experience. Ability to interpret and apply financial procedures and regulations preferred. Previous experience with hospital information systems or word processing preferred.

For every 100 additional pounds, assistance will be required from another healthcare worker. 20 lbs. of force is needed to push a 400 lb. patient in a wheelchair on carpet. 25 lbs. of force is required to push a stretcher with a patient with one hand. Ability to maintain 20/40 vision, corrected, in one eye or with both eyes. (Continuous) Ability to see and recognize objects close at hand or at a distance. (Continuous) Ability to match or discriminate between colors. (Continuous) Ability to determine distance/relationship between objects; depth perception.
