Orthopedic At-Home Physical Therapist

JAG Physical Therapy seeks an Orthopedic At-Home Physical Therapist to deliver 1:1 rehab services to patients in their homes across New Jersey. You will evaluate orthopedic conditions, develop and adjust evidence-based treatment plans, provide hands-on therapy, and educate patients and families on home exercise programs and injury prevention. Collaborate closely with physicians and the JAG team, maintain accurate EMR documentation, and contribute to a culture of learning and clinical excellence.
