Residence Home Care continues to grow and is looking for a FT and PRN Occupational Therapists to join our team. The territory for this position is eastern Cuyahoga County (Garfield, Mayfield, Beachwood, Bedford, Warrensville, Shaker Hts, Cleveland Hts) and Lake County (Mentor, Willoughby, Wickliffe, WIllowick, Painesville). We are looking for someone who can help improve the well-being of our patients and their families and has the heart and skills to work with home bound patients. Qualifications: · Ability to perform therapy evaluations and OASIS documentation · Work with other disciplines to give the best care needed for each patient. · Help decrease the need for re-hospitalization by being actively involved in the patient's plan of care to assess any changes that may be needed. · When appropriate teach and counsel family members about care and processes for disease/injuries. · Attend orientation, meetings, and CEUs |
