PACE KC Licensed Practical Nurse

As a Licensed Practical Nurse (LPN) for PACE KC, you will be part of a team of caregivers to provide care for the people who need it most. Under the direction and supervision of the Medical Clinic Manager, provides general and skilled nursing care to participants, executes interventions in accordance with the plan of care, implements primary care orders, evaluates participant complaints in person and/or by telephone, develops appropriate plan of care, provides participant, caregiver and family education and communicates participant's condition to primary caregivers, physician and care team.

Primary Functions: 

1. Function as a member of the Care Team. Maintains regular attendance at, and participants in Care Team meetings; communicates participant changes, collaborates on plan of care decisions and coordination for twenty-four (24) hour care delivery. Effectively communicates with team mates during meetings or one-on-one conversations.
2. In coordination with the Medical Care Manager and Registered Nurse and other nursing staff, develops and implements a plan of care to meet the needs of participants to remain living safely in the community for as long as possible in alignment with their goals of care. Maintains aligned, comprehensive plan of care for all participants living in residential facilities and coordinates care between facility and Care/Interdisciplinary Team.
3. Perform general and skilled nursing care to participants as assigned. Implements the plan of care and physician orders in coordination with the Care/Interdisciplinary Team. Observes, records, and reports participant's condition and reaction to drugs and treatments to providers. Triages participants to determine priority of care and assists in the treatment, examination, and testing of participants.
4. Participate in nursing on-call rotation to provide after-hours and weekend assistance and care, as needed, to PACE KC participants.
5. Receive participant complaints, triages, and responds to participant needs.
6. Review and signs off orders and disseminates orders and instructions to appropriate staff and contracted personnel. Maintains and reviews participant records, charts, and other pertinent information. Completes documentation and charting in accordance with organizational policy.
7. Working with the Medical Care Manager, performs care coordination with outside providers and facilities including hospitals, nursing homes, assisted living facilities, and physician/provider offices. May accompany participants to specialist appointments or make hospital or other facility visits when necessary for coordination of participant care.
8. Provide treatment instructions and nursing education for participants and family.
9. Maintain accountability for high customer service standards and monitors, evaluates, and recommends opportunities for improved participant and family/caregiver satisfaction.
10. Manage and reviews participant's records, charts, pertinent information and staff's documentation. Documents as needed in medical records according to PACE KC standards.

1. Maintain the stability and reputation of PACE KC by ensuring all activities and operations are performed in compliance with local, State, and Federal laws, regulations, and contractual
   requirements.
2. Establish and maintain a productive working relationship with relevant contracted providers.
3. Responsible for adherence to program cultural standards (Fundamentals) including supporting the development and integration of these standards within the team through modeling, coaching, and accountability.
4. Follow and ensure that clinical team follows all organizational policies and procedures and CDC and Occupational Safety and Health Administration (OSHA) safety guidelines.
5. Educate staff in regard to care and treatment for the elderly as needed.
6. Act within the scope of his or her authority to practice.
7. Handle potentially infectious specimens with appropriate biohazard precautions, and practices Universal precautions.
8. Protect privacy and maintain confidentiality of all company procedures, results and information about employees, participants and families.
9. Participate in continuing education classes and any required staff and training meetings.
10. Maintain professional affiliations and any required certifications.
11. Perform other duties as required or requested.

8. Ability to apply creative problem-solving skills to complex issues. Ability to plan, prioritize, and follow-up on responsibilities.
9. Ability to foster collaborative working relationships. Ability to supervise, direct, coach, and develop direct reports and larger team. Demonstrated ability to work with a diverse team and flourish in a flexible work environment.
10. Skilled in establishing and maintaining effective working relationships with participants, medical staff, staff members and family caregivers.
11. Knowledge of infection control practices and quality assurance and improvement.
12. Knowledge of the PACE KC model and philosophy of care.
13. Willingness to provide on-call coverage within the nursing rotation.

Qualifications and Experience

1. Must have a valid driver's license, proof of insurance and have means of transportation.
2. Must have medical clearance for communicable diseases and up-to-date immunizations before having
   direct participant contact.
3. Education: Graduate of a school of professional nursing. Cardiopulmonary Resuscitation (CPR)
   and intravenous (IV) certification required.
4. Licensure: Unencumbered license as a Licensed Practical Nurse (LPN) licensed as required by
   state law.
5. Experience: A minimum of one-year (1) experience in working with the frail or elderly
   population.

Successful Candidates will have:

• Graduated from an accredited school of practical nursing.
• Licensed to practice in the State of Missouri and/or Kansas.
• Basic Cardiovascular Life Support (BCLS), certified.
• Excellent communication skills, customer service oriented and exhibit the ability to work with people (staff, visitors and clients).
