Job Summary:
The Patient Logistics Coordinator is responsible for assisting the Medical Director of Emergency Medicine including but not limitied to Transfer Center operations, Patient Placement, and patient flow required to provide appropriate, effective, and efficient patient care. The Patient Logistics Coordinator is empowered to remove barriers and bottlenecks related to transferring of patients, patient placement, and flow. Works in collaboration with Transfer Center and Patient Placement coordinators and communicates with Nursing staff, Nurse Managers, NCCs, Support Services, Administrative Supervisors, and Physicians. Interacts with Senior Director of Clinical Operations of Telehealth Center.
Responsibilities:
- Provides direct oversight of each incoming transfers; intervening for any delays to efficient transfer and ensuring appropriate service is advised of incoming transfers. Escalates to Medical Director of WMCHealth Emergency Medicine as appropriate. Provides handoff of transfers
- Assists with triage of transport priorities for incoming transfers in collaboration with the Medical Director to assist the patient placement coordinators in the placement of elective and emergent admissions as well as transfers in accordance with clinical standards and guidelines ensuring all patients are admitted to or transferred to the most appropriate available bed in conjunction with Senior Director of Clinical Operations, Telehealth Center
- Clinically knowledgeable of disease entitites, epidemiological issues, and circumstances that have implications for the placement of patients
- Assists with review of blocked beds and appropriate admission and level of care criteria
- Reviews patients conditions
- Tracks bed availability, maintaining appropriate logs/records
- Performs walking rounds on nursing units to identify and discuss barriers to discharge and confirm available beds.
- Works directly with patient placement coordinators to monitor bed availability on all unit and pending discharges assessing the ability to meet needs of cath lab, PACU, ED, elective admissions, and incoming transfers.
- Assists with distribution of transfer pateints across the network
- Acts as a resource to patient placement staff as to the type of patient each unit can accept using policies and procedures as a guide
- Regularly communicates with and provides status reports to the Medical Director regarding impending need for additional capacity, transfer center , ED, and PACU holds and any potential delays.
- Performs quality assurance/improvement projects and data analysis
- Other duties as assigned
Qualifications/Requirements:
Experience:
Minimum 3 years experience EMT-P, Minimum 2 years Interfacility Transport experience, required.
Education:
High School Dipolma, required. Associates Degree, preferred. Or Equivalent combination of experience and education may be substituted for degree requirement.
Licenses / Certifications:
- EMT-P, preferred
- NRP, preferred
- CC-P or FP-, preferred
