The Patient Navigator - Urology functions on a multidisciplinary team as an advocate and educator for prostate cancer patients. Participates in coordination of care and acts as a liaison between the patients, families, caregivers and providers to optimize patient outcomes. This position also contributes to the knowledge base of the healthcare community as liaison throughout the network and in the community. Provides assistance to the physicians and practitioners with patient care, administrative, environmental and organizational tasks.

Schedule:

Monday - Friday, 8:00am to 4:30pm, eight hour shifts

No evenings, weekends or holidays!

Essential Duties and Responsibilities:

  • Coordinate patient care including scheduling, care transitions, communication, and referrals for services in collaboration with physicians and the broader care delivery team across the continuum of care
  • Assess patients' current and future needs across a broad spectrum of medical, psychosocial, and financial aspects and make referrals to internal and external resources as appropriate
  • Interface with other healthcare teams for appropriate referrals and services
  • Provides and reinforces education to patients, families, and caregivers about diagnosis, treatment options, side effect management, and post- treatment care and survivorship
  • Promotes awareness of clinical trials to patients, families, and caregivers
  • Navigate patient through diagnostic studies
  • Run diagnostic clinic procedures
  • Assist with in office treatments- including but limited to giving injections and catheter placement
  • Ensures documentation of patient encounters and provided services
  • Continually assess patients' treatment progress and educational needs
  • Maintain ongoing relationship with patients and their families throughout treatment
  • Facilitate language translation, interpretation services, financial assistance, transportation, and other patient assistance required by the patients treatment plan.
  • Build partnerships with local agencies and groups for referrals
  • Document all patient coordination activities, education, and other interactions in designated inpatient and outpatient electronic health record systems
  • Greets patients and assists with patient flow;
  • Assists with preparatory activities such as taking and documenting vital signs including blood pressure, pulse, temperature, height and weight, and communicates abnormalities to physician or practitioner;
  • Assists physicians and/or practitioners with patient care including minor surgical procedures and/or general medical procedures;
  • Reviews and documents patient medical history, assists with ensuring encounter forms are complete;
  • Collects and prepares lab specimens;
  • Electronically submit prescriptions/refills to pharmacies as directed;
  • Prepares patient rooms, cleans/sterilizes instruments;
  • Answer patient/pharmacy calls, take messages for physicians, relay lab/test results and instructions to patients as directed, relay hospital consult information to physicians, and other administrative duties as assigned;
  • MU2/PQRS reporting as instructed;
  • Follow all health care privacy and safety rules and adheres to Dayton Physicians Core Values;
  • Willing to travel to other locations for business needs

Required Education and/ or Experience:

  • Requires a High School Diploma or GED and;
  • Two (2) year degree from a college or technical school, and;
  • Medical Assistant Certification or License Practice Nurse (LPN)
  • One (1) year of previous experience as a Medical Assistant or LPN;
  • 2-4 years' experience is preferred.
  • Current CPR certification, if not it must be obtained within 60 days of employment and kept current thereafter.

General Skill Requirements:

  • Excellent oral and written communication
  • Working knowledge of Microsoft Office, including Word, Excel and Outlook.
  • Experience working with a Practice Management system.
  • Attendance and punctuality are essential requirements of this position to provide excellent customer service to both our internal and external stakeholders.
