Job Summary

Ensures patients receive the best possible care through effective care coordination, communication, and follow-up. Ensuring timely and accurate scheduling of appointments, supporting the clinical team in gathering and documenting patient data, and verifying documentation to ensure it meets all guidelines for appointment completion is essential. The incumbent proficiently manages telephonic encounters, with a focus on clear, professional, and compassionate communication. Serves as a primary point of contact for patients and other stakeholders. Collaborates with the care team to ensure comprehensive patient care coordination. Supports community relations and actively participates in organizational and department goals/objectives. This role emphasizes effective communication as the cornerstone of building patient engagement and ensuring the smooth flow of information between patients, caregivers/families, and the clinical team to ensure optimal care delivery and patient/caregiver satisfaction.

Qualifications

  • High School or equivalent Required
  • Associate's degree in a healthcare-related field Preferred
  • 1-3 years experience in a medical setting, in role such as administrative assistant, front desk staff, or patient service representative Preferred
  • Experience managing patient records and documentation. Required
  • Ability to read, write, and communicate in English.
  • Ability to communicate in Spanish is preferred.
  • Familiarity with medical terminology and understanding of healthcare processes.
  • Strong background in customer service, and skilled in handling patient inquiries, complaints, and concerns effectively.
  • Familiarity with electronic health record (EHR) systems and medical billing software preferred.
  • Competence in using office software (e.g., Microsoft Office Suite) for documentation and communication.
  • Experience in identifying issues and implementing solutions to improve patient intake processes and workflows preferred.
  • Knowledge of community supportive services in the area (Area on Aging, Meals on Wheels, transportation, etc.) and drug assistance programs are preferred.
  • Certification in patient navigation or as a community health care Preferred
