Key Duties:
Direct ADL Support: Provide expert assistance with activities of daily living (ADLs), including bathing, dressing, grooming, toileting, and meal assistance.
Clinical Monitoring: Measure and accurately document vital signs; remain vigilant for any changes in a participant's physical or mental condition and report updates to the interdisciplinary team.
Mobility & Safety: Assist participants with safe transfers, boarding and exiting transportation vehicles, and acting as a medical escort for appointments.
Nutritional Support: Coordinate meal services to ensure participants receive correct dietary plans and provide feeding assistance when necessary.
Engagement & Activities: Partner with the Engagement team to facilitate participant activities, fostering a vibrant and social day center environment.
Facility Stewardship: Maintain personal care and grooming supplies; ensure participant areas remain clean, orderly, and pleasant between activities and meals.
Administrative Integrity: Complete all required clinical documentation in a timely, accurate manner using Electronic Medical Record (EMR) systems.

Education: One of the following is required:
High School Diploma or Equivalency.
California DPH – Home Health Aide (HHA) or Certified Nursing Assistant (CNA) certification.
Experience: Minimum of one (1) year of experience providing personal care specifically to a frail or elderly population.
Certification: Must possess a valid CA Food Handler’s Certificate.
Technical Skills: Experience navigating and documenting within Electronic Medical Record (EMR) systems.
Soft Skills: Excellent organizational and communication skills with the ability to prioritize tasks in a fast-paced setting.
Advanced Credentialing: Active California certification as a Home Health Aide (HHA) or Certified Nursing Assistant (CNA) is highly preferred.
Cultural Competency: Demonstrated experience and sensitivity in working with individuals from diverse backgrounds and cultures.
Independence: Proven ability to follow complex directions and work effectively with minimal supervision.

Work Type: 3-Month Contract
Schedule: Monday – Friday | 8:00 AM – 4:30 PM
Flexibility: Ability to work a varied schedule including occasional evenings, weekends, or overtime as business needs evolve.
