PCA

JOB SUMMARY:

The PCA provides personal and hygiene care to ill or handicapped patients in the home or other appropriate setting. The Home Health Aide follows the nursing care plan and reports to the primary nurse and/or the nurse designated as the Home Health Aide Program Nurse.

Qualifications:

A. Successful completion of a training and competency program for HHAs approved by the State of Connecticut Department of Health Services.

Physical and Mental Effort:

Medium work exerting 20 to 50 pounds of force occasionally (up to 1/3 of the time), and /or 10 to 25 pounds of force frequently (1/3 to 2/3) of the time, and/or greater than negligible force up to 10 pounds consistently.

This position requires daily driving an automobile varied distances to client homes and the ability to enter/exit an automobile numerous time daily. Must have the ability to enter buildings using varied levels of stairs to provide service to clients. The Home Health Aide always presents a professional attitude and appearance and maintains confidentiality of all client information.

Job Duties

1. Provides personal and hygiene care to patient as specified in the nursing care plan.

1.1 Provides personal and hygiene care.

a. Complete or partial bathing.

c. Mouth and denture care.

d. Routine skin care and inspection

e. Shaves male patient.

f. Recognizes need for additional hygiene.

1.2 Reminds patient to take pre-poured medications.

a. Follows specific written instructions for patient care.

b. Recognizes obvious problems and reports to primary nurse.

1.3 Assists with treatments self-administered by patient.

1.4 Assists with special therapy plans.

a. Receives instructions from therapist.

b. Follows written plan established by therapist.

c. Assists with use or application of special adaptive equipment as instructed by therapist.

1.6 Assists patient to maintain or attain maximum functional goals determined by patient care plan.

a. Increase or maintain ambulation status.

b. Participates in diverting activities.

c. Provides emotional support.

d. Observes physical and emotional condition and reports to primary nurse or HHA supervisor.
