Osborn Family Health Center located in Camden South Jersey is a few steps away from Philadelphia and a short drive to the Jersey shores and Pennsylvania suburbs. We are a multispecialty practice encompassing Family Medicine, Pediatrics, OBGYN and clinics. The center was established in 1974 in a brownstone house that was donated by Dr. Edward Osborn, the first Chief of Surgery at Our Lady of Lourdes Medical Center. For over 30 years, Osborn Family Medical Centers, has provided primary healthcare services to local communities, including residents and undocumented immigrants. We strive to provide high quality healthcare that reduces the financial and cultural barriers to meet the needs of our culturally diverse community. The Osborn Family Medicineteam consists of dedicatedphysicians, nurse practitioners, RNs, and clinical social workers. We collaborate to meet the healthcare needs of each one of our patients.
Osborn Family health center is a Federally Qualified Health Center (FQHC). APNs working for an FQHC are eligible for up to $50,000 through the Federal Loan Repayment Program.

We are committed to providing comprehensive, high-quality care to underserved communities. As a Patient Centered Medical Home (PCMH), our team is passionate about improving health outcomes and ensuring access to compassionate pediatric care.
Responsibilities include:
Delivering primary pediatric care including well-child visits, immunizations, developmental screenings, and acute care
Providing behavioral health support for adolescents, including screening and early intervention
Participating in theReach Out and Readprogram to promote early literacy and family engagement
Collaborating with community partners and local organizations to support wraparound services for families
Educating patients and caregivers on health maintenance, disease prevention, and developmental milestones
Working closely with an interdisciplinary team including physicians, behavioral health providers, and care coordinators
Maintaining accurate and timely documentation in the electronic health record
Participating in quality improvement and population health initiatives
Practicing within the full scope of licensure and certification as a Nurse Practitioner, while recognizing and respecting professional boundaries and collaborative protocols
Schedule:
Monday through Friday
One late evening per week
No weekends
On-call responsibilities included
Scheduled Admin Time
Qualifications:
Licensed Pediatric Nurse Practitioner in the state of New Jersey
CDS, DEA
Board certified (PNP-PC)
Experience in community health or underserved populations preferred
Two years minimum practicing as a nurse practitioner
