Job Description
Position Summary
Provide home-based occupational therapy evaluations, treatment and discharge planning for adult and geriatric patients. Work on a per diem basis with flexible shifts; travel to patients' homes across Palm Beach County is required.
Key Responsibilities
- Conduct skilled OT evaluations in patients' homes; develop measurable, reimbursable care plans
- Deliver skilled therapeutic interventions (ADLs, IADLs, upper-extremity function, home safety, mobility, splinting, adaptive equipment training)
- Complete timely, accurate documentation meeting Medicare, state, and agency standards
- Communicate with interdisciplinary team, physicians, and family/caregivers about progress and discharge needs
- Maintain caseload within the scope of practice and follow agency policies
Qualifications
- Licensed Occupational Therapist (OTR or OTR/L) in the state of Florida
- Current BLS/CPR certification
- Minimum 1 year of clinical experience in home health, skilled nursing, outpatient or acute care
- Valid driver's license, reliable vehicle, current auto insurance, and willingness to travel throughout Palm Beach County
- Strong documentation skills and familiarity with home health Medicare regulations and OASIS preferred
- Excellent communication, clinical reasoning, and patient education skills
