Per Diem Physical Therapist

Job Description:
We are currently seeking a dedicated and compassionatePhysical Therapist (Per Diem)to join our home health team inLynn and Dorchester, MA. This is a flexible, per-visit opportunity ideal for clinicians who enjoy working independently and providing high-quality care in patients’ homes.

Responsibilities:

• Provide physical therapy services to patients in their homes

• Evaluate patient needs and develop individualized treatment plans

• Complete accurate and timely documentation, including OASIS assessments

• Educate patients and families on treatment plans and progress

• Collaborate with the interdisciplinary care team to ensure optimal patient outcomes

Qualifications:

• ActivePhysical Therapist (PT) licensein Massachusetts

• CPR certification(current)

• OASIS experiencerequired

• Home health experiencepreferred

• Reliable transportation and willingness to travel within assigned areas

• Strong communication and documentation skills

• English proficiency required
