We are seeking a compassionate and skilled Physical Therapist (PT) to join our team in Havertown, Pennsylvania on a per diem basis. This is an excellent opportunity for a dedicated healthcare professional who is passionate about helping patients regain mobility, improve function, and achieve greater independence.

As a Physical Therapist, you will work collaboratively with an interdisciplinary healthcare team to develop and implement individualized treatment plans that support optimal patient outcomes.

Core Responsibilities

Patient Evaluation

• Assess patients' functional status, strength, mobility, balance, and physical limitations.
• Develop individualized treatment plans tailored to each patient's rehabilitation goals and needs.

Therapeutic Interventions

• Provide skilled physical therapy procedures and interventions to improve movement, strength, function, and independence.
• Monitor patient progress and adjust treatment plans as necessary to achieve desired outcomes.

Education

• Teach patients, families, and caregivers safe exercises, adaptive techniques, injury prevention strategies, and proper use of assistive devices.
• Encourage patient participation in the rehabilitation process to support long-term recovery.

Documentation

• Maintain accurate and timely documentation of evaluations, treatment plans, patient progress, and outcomes.
• Ensure compliance with facility policies, professional standards, and regulatory requirements.

Team Collaboration

• Collaborate with physicians, nurses, case managers, and other healthcare professionals to coordinate patient care and support successful transitions of care.
• Participate in interdisciplinary treatment planning and discharge recommendations.

Qualifications & Requirements

✅ Bachelor's or Master's Degree from an accredited Physical Therapy program

✅ Minimum of 1 year of Physical Therapy experience required, preferably in an acute care setting

✅ Active Pennsylvania Physical Therapist License in good standing

✅ Current Basic Life Support (BLS) certification required within 30 days of hire

✅ Strong assessment, treatment planning, and patient education skills

✅ Ability to stand, bend, kneel, and safely lift or move patients and equipment weighing up to 50 pounds
