Overview:

We're Hiring: Per Diem Physical Therapist

Location: Toms River NJ Bancroft Outpatient Program

Type: Non Exempt, PRN

Hourly Rate: $58 hr.

Hours: Day Program - Monday though Friday Day Hours

 

Position Summary:

Provides direct, consultative, and supportive diagnostic and therapeutic physical therapy services to persons served and staff members to enhance the individual potential for learning and to meet the specific rehabilitation needs of persons served. 

 

Responsibilities:
  • Provide direct and consultative physical therapy services, including evaluations, treatment planning, and interventions, in compliance with APTA standards, state regulations, HIPAA, and organizational policies.

  • Conduct physical therapy assessments to determine individual needs, functional abilities, limitations, and rehabilitation goals within residential and program settings.

  • Develop, implement, and modify individualized physical therapy treatment plans based on assessment findings and ongoing progress.

  • Establish measurable, functional, and achievable therapy goals that support independence and daily living skills.

  • Deliver individual, group, and consultative therapy services while promoting independence and functional mobility.

  • Recommend, demonstrate, and train individuals and staff on the proper use of adaptive equipment, positioning techniques, and therapeutic interventions.

  • Collaborate with interdisciplinary team members, families, guardians, and program staff to support coordinated care and ensure continuity of services.

  • Provide guidance on environmental adaptations, functional strategies, and adaptive equipment to enhance participation and progress.

  • Maintain accurate, timely, and professional documentation, including evaluations, progress notes, and treatment summaries, in accordance with IPP and regulatory requirements.

  • Participate actively in team meetings and the Trans-Disciplinary Team (TDT) process, communicating assessment results and treatment outcomes to all relevant stakeholders.

  • Provide training and education to staff and individuals served regarding physical therapy goals, techniques, and treatment plan implementation.

Qualifications:

Education & Experience: 

Master’s degree in Physical Therapy and a current N.J. State Physical Therapist license required.  Minimum of one year of work experience in physical therapy preferred, working with neurologically-impaired individuals.
