About the Role:
Karing Hearts Home Health is looking for a compassionate and skilled Per Diem RN to join our dedicated home health team in Imperial, CA. This is a fantastic opportunity for a registered nurse who values flexibility while making a meaningful difference in patients' lives right in their own homes. Responsibilities:

• Conduct skilled nursing assessments and develop individualized patient care plans in a home health setting
• Administer medications, wound care, IV therapy, and other clinical treatments as ordered
• Monitor and evaluate patient progress, adjusting care plans as needed
• Educate patients and family members on disease management, medications, and self-care techniques
• Collaborate with physicians, therapists, and interdisciplinary team members to coordinate patient care
• Accurately document clinical notes and patient records in compliance with regulatory standards
• Ensure patient safety and comfort while promoting independence and quality of life

Requirements:

• Active California Registered Nurse (RN) license in good standing
• Minimum 1 year of clinical nursing experience; home health experience preferred
• Current CPR/BLS certification required
• Valid California driver's license and reliable transportation
• Proficiency with electronic medical records (EMR) and clinical documentation
• Strong communication, time management, and critical thinking skills
• Compassionate, patient-centered approach with the ability to work independently
