Responsibilities:- Conduct skilled nursing assessments and develop individualized patient care plans
- Administer medications, wound care, IV therapy, and other clinical treatments in the home setting
- Monitor and evaluate patient progress, adjusting care plans as needed
- Educate patients and family members on disease management, medications, and home safety
- Collaborate with physicians, therapists, and interdisciplinary team members to coordinate care
- Accurately document patient visits, clinical findings, and care plans in compliance with regulatory standards
- Ensure adherence to Medicare, Medi-Cal, and home health agency guidelines
Requirements:- Active California Registered Nurse (RN) license in good standing
- Minimum 1 year of clinical nursing experience; home health experience preferred
- Current CPR/BLS certification
- Valid California driver's license and reliable transportation
- Familiarity with OASIS documentation and Medicare home health regulations is a plus
- Strong clinical assessment, critical thinking, and communication skills
- Compassionate, self-directed, and comfortable working independently in the field
