About the Role:
Karing Hearts Home Health is looking for a compassionate and skilled Per Diem RN to join our Home Infusion team in San Bernardino, CA. This is a fantastic opportunity for a motivated nurse to make a meaningful impact by delivering high-quality infusion therapy directly to patients in the comfort of their own homes. Responsibilities:

• Administer IV infusion therapies including antibiotics, TPN, hydration, and specialty medications in the home setting
• Perform comprehensive patient assessments and monitor responses to infusion treatments
• Educate patients and caregivers on infusion therapy, medication management, and safety protocols
• Insert, maintain, and troubleshoot peripheral and central venous access devices (PICC lines, ports, central lines)
• Accurately document patient care, clinical findings, and treatment plans in compliance with regulatory standards
• Collaborate with physicians, pharmacists, and the care team to coordinate and adjust patient treatment plans
• Respond to patient concerns and on-call needs in a timely and professional manner

Requirements:

• Active and unrestricted California RN license required
• Minimum 1-2 years of IV/infusion therapy or home health nursing experience preferred
• Proficiency in peripheral IV insertion and central line access (PICC, port, tunneled catheters)
• Strong clinical assessment and critical thinking skills
• Valid California driver's license, reliable transportation, and current auto insurance
• CPR/BLS certification required
• Excellent communication skills and ability to work independently in a home care environment
• Compassionate, patient-centered approach with strong organizational skills
