About the Role:
Karing Hearts Home Health is looking for a compassionate and skilled Per Diem RN to join our Home Infusion team in Ventura, CA. This is a fantastic opportunity for an experienced nurse to make a meaningful difference in patients' lives by delivering high-quality infusion therapy in the comfort of their own homes. Responsibilities:

• Administer IV infusion therapies including antibiotics, TPN, chemotherapy, and hydration in patient homes
• Perform thorough patient assessments and monitor responses to infusion treatments
• Educate patients and caregivers on infusion therapy, medication management, and home care protocols
• Maintain, troubleshoot, and manage IV access devices including PICCs, ports, and central lines
• Accurately document patient care, clinical findings, and treatment plans in compliance with regulatory standards
• Collaborate with physicians, pharmacists, and care team members to coordinate patient treatment plans
• Respond to patient concerns and on-call needs as required per diem schedule

Requirements:

• Active California RN license in good standing
• Minimum 1-2 years of IV infusion or home health nursing experience preferred
• Proficiency in peripheral and central line access, including PICC and port management
• Strong clinical assessment and critical thinking skills
• Excellent communication skills and ability to work independently in a home setting
• Valid California driver's license and reliable transportation required
• CPR/BLS certification required
