In collaboration with the medical and facility staff, coordinates multidisciplinary care for the Stroke and Sepsis patients. Works in tandem with the Medical Directors of the Stroke and Sepsis program. Roles and responsibilities include but are not limited to the following:
Minimum Qualifications: Bachelor of Science in Nursing, master’s degree (preferred). Minimum of three (3) years of relevant experience in acute care setting overseeing and/or performing data abstraction, analysis, and process improvement. Basic Life Support - BLS/BCLS (American Red Cross or American Heart Association) - Course/Certification (required) Advanced Life Support - ALS/ACLS - Course/Certification. Certification in emergency nursing preferred. Current experience in caring for neurology patients, Current National Institute of Health Stroke Scale (NIHSS) Certification and 8 hours of stroke education annually required. Knowledge, Skills & Abilities: Knowledge of clinical documentation systems/EHR. Experience in a quality improvement department setting preferred. Detail-oriented with strong organizational and analytic problem-solving skills, with exceptional verbal and written communication skills. Competent in the use of Microsoft Office Word and PowerPoint, and Excel. Knowledge of, or experience in, the delivery of patient care needed to perform the duties of the position. Experience interacting with regulatory surveyors preferred. Maintains knowledge of measure specifications and data definitions and facilitates process improvements to meet requirements.
