Responsibilities

  • Provide high-quality physical therapy services to patients on-site.

  • Assess patient needs and develop individualized treatment plans.

  • Meet or exceed weekly/monthly productivity and outcomes targets.

  • Record progress and outcomes in patient records with accuracy.

  • Monitor patient response and adjust treatments accordingly.

  • Utilize data and pipelines to identify potential patient referrals.

  • Collaborate with physicians and teammates to coordinate care.

  • Educate patients and caregivers on home exercises and self-management.

  • Participate in ongoing professional development and certifications.

  • Maintain clean and compliant clinical environment and equipment.

  • Communicate effectively with leadership to report progress and obstacles.


Experience/Qualifications

  • Licensed physical therapist with current state licensure and CPR certification

  • Demonstrated ability to assess patients and create individualized treatment plans

  • Accurate documentation of progress and outcomes in electronic medical records

  • Proven productivity mindset with ability to meet weekly and monthly targets

  • Ability to monitor responses and adjust therapies promptly based on data

  • Collaborative care approach with physicians and teammates

  • Commitment to ongoing professional development and certifications

  • Experience using data pipelines or analytics to identify referrals

  • Patient and caregiver education on home programs and self-management

  • Maintain compliant clinical environment and equipment, including infection control

  • Strong communication with leadership; reporting progress and obstacles timely
