Inspired Options Inc. is seeking an experienced, organized, and detail-oriented Person-Centered Plan (PCP) Coordinator to support the delivery of high-quality, person-centered services to individuals with intellectual and developmental disabilities.

This position is ideal for a human services professional with strong knowledge of Maryland Developmental Disabilities Administration (DDA) services, Person-Centered Plans, Service Implementation Plans (SIPs), service authorizations, documentation, and compliance.  The PCP Coordinator will serve as a central point of coordination among program participants, Coordinators of Community Services (CCSs), families and guardians, program leadership, nursing staff, Direct Support Professionals (DSPs), billing and quality assurance teams, and other stakeholders.

As the PCP Coordinator, you will manage and monitor the full PCP process and help ensure each participant's approved services are translated into measurable, person-centered supports.

Key responsibilities include:

  • Manage the annual PCP calendar and coordinate preparation meetings, annual PCP meetings, revisions, quarterly reviews, and interdisciplinary team meetings.
  • Review PCPs for approved outcomes, goals, services, staffing supports, health and safety needs, behavioral supports, community participation, and documentation requirements.
  • Develop, update, and maintain Service Implementation Plans (SIPs) that align with approved PCPs and service authorizations.
  • Translate PCP outcomes and goals into clear, measurable implementation strategies for DSPs and program staff.
  • Develop and maintain systems for tracking participant outcomes, goals, progress, and required documentation.
  • Review service notes and program documentation to identify gaps in goal implementation, service delivery, and outcome progress.
  • Review and monitor service authorizations, approved hours, units, effective dates, expiration dates, and service changes within LTSSMaryland.
  • Collaborate with appropriate staff to maintain accurate service and authorization information within CIMS.
  • Develop and coordinate Detailed Service Authorization Tool (DSAT) information based on participant needs and approved/requested services.
  • Work with nursing staff to ensure health, safety, HRST, medication, and delegated nursing considerations are appropriately incorporated into service planning.
  • Support new participant admissions by reviewing PCPs, authorizations, DSAT information, health and safety needs, and service requirements.
  • Train DSPs, House Managers, Program Coordinators, and other staff on PCP outcomes, SIP implementation, documentation standards, and service expectations.
  • Conduct routine quality assurance reviews and audits of PCPs, SIPs, service notes, authorizations, outcome documentation, and related records.
  • Identify documentation or authorization discrepancies and coordinate timely resolution with program leadership, CCSs, nursing, billing, and other stakeholders.
  • Maintain organized, current, confidential, and audit-ready participant records.
  • Support documentation and authorization readiness for billing and regulatory reviews.
  • Assist with PCP and SIP updates following hospitalizations, significant incidents, behavioral changes, health changes, or other changes in participant needs.

Required Qualifications

  • Bachelor's degree in human services, social work, psychology, healthcare administration, public health, education, business administration, or a related field preferred. Equivalent relevant experience may be considered.
  • Minimum of 3–5 years of relevant experience in developmental disabilities services, human services, healthcare administration, program coordination, compliance, case management, or operations.
  • Experience supporting individuals with intellectual and developmental disabilities.
  • Working knowledge of the Maryland DDA service delivery system.
  • Experience with Person-Centered Plans, Service Implementation Plans, service authorizations, documentation requirements, and interdisciplinary team coordination.
  • Experience using electronic health record, case management, or documentation systems.
  • Strong ability to manage deadlines, records, calendars, documentation, and multiple priorities.

Strongly Preferred

  • Direct experience working with a Maryland DDA provider.
  • Familiarity with LTSS Maryland, CIMS, DSAT, HRST, and DDA billing-related processes.
  • Experience developing or auditing Service Implementation Plans.
  • Experience with quality assurance, regulatory compliance, documentation audits, or billing readiness.
  • Experience training DSPs or other human services professionals.
  • Experience with EHR workflows, documentation templates, and data-tracking systems.
  • Master's degree in a related human services, healthcare, education, or administrative discipline.

  • Proficiency with EHR systems, spreadsheets, databases, and digital documentation platforms.
  • Ability to maintain confidentiality and professional boundaries.
  • Ability to work independently while effectively collaborating within an interdisciplinary team.
