Job Summary

The Day Center Personal Care Assistant (PCA) is responsible for providing safe delivery of personal care services to participants at the day center, as directed. In addition, the PCA supports the Nutrition, Engagement (Activities), and Home Health departments as needed, once provided appropriate training. The role requires effective collaboration with colleagues and stakeholders to promote team culture and mission.

Job Responsibilities

  • Provide personal care and assistance with activities of daily living (ADLs) to participants, including escorting, toileting, bathing, dressing, grooming, assisting with meals, laundry, etc.
  • Measure and document participants' vital signs as instructed.
  • Assist participants with boarding and exiting from transportation vehicles and act as a medical escort when instructed.
  • Observe activity on the day center floor to assure participant safety.
  • Assist with meal service, ensuring participants receive the correct meals and provide support with feeding when indicated.
  • Assist participants with safe transfer in and out of their homes and act as a medical escort on vehicles and to medical appointments as assigned.
  • Maintain personal care supplies such as incontinence supplies and personal grooming items.
  • Assist the Engagement team with participant activities.
  • Assist the Home Care team as needed.
  • Clean participant areas in the day center between meals and activities.
  • Remain alert and update the supervisor or any member of the IDT promptly of any changes in participants' condition.
  • Be legally authorized to perform in position location, required.
  • Only act within the scope of his or her authority to perform.
  • Must be willing and have the ability to work a varied schedule that may include evenings, nights, weekends, and overtime.
  • Complete all required documentation in a timely and accurate manner.
  • Protect privacy and maintain confidentiality of all company procedures and information about team members, participants, and families.
  • Follow company policies and procedures and participate in any required Quality Improvement activities, staff training, and meetings.
  • Communicate regularly with Supervisor and team regarding workload and priorities.
  • Timely completion of all mandated trainings and education.
  • Timely completion of all mandated occupational health screenings as needed.
  • Exercise flexibility in performing assignments as business needs evolve.
  • Other duties as assigned.

Required Skills & Experience

  • Experience with electronic medical record systems.
  • Minimum of one (1) year of experience providing personal care to a frail or elderly population.
  • Ability to follow directions from the supervisor and communicate needs effectively.
  • Excellent organizational and communication skills.
  • Ability to work independently with minimal supervision.
  • Demonstrated ability to prioritize in a fast-paced environment.
  • Experience and competency working with people from diverse backgrounds and cultures.

Schedule Notes

Monday-Friday 8-hour shift. Must be available between the hours of 6:30 AM and 6:30 PM. Candidates must have reliable transportation.
