Two Direct Hire Opportunities for Inpatient Oncology/Hematology Pharmacists
Immediate openings are available for inpatient oncology/hematology pharmacists serving a leading healthcare facility in the Des Plaines, IL area. These direct hire positions are for pharmacists who are passionateabout specialized acute care in oncology, hematology, gynecologic oncology, and stem cell transplantation. The role involves working closely with interdisciplinary teams to optimize medication regimens through daily rounds and patient counseling.
Location:
Schedule:- Typical inpatient schedule including days, evenings, and rotating weekends
Responsibilities:- Participate in interdisciplinary rounds to assist with medication regimen design, implementation, and monitoring.
- Review drug therapies for appropriate use, dosage, and interactions.
- Reconcile home medications upon patient admission.
- Communicate medication clarifications with healthcare teams and document changes in records.
- Provide patient and family counseling on medication adherence and side effects, prior to discharge.
- Deliver pharmacokinetic consultations and monitor therapies including vancomycin and aminoglycosides.
- Monitor and report adverse drug reactions and medication errors.
- Review inpatient chemotherapy orders across oncology specialties.
- Support nursing staff by answering medication-related inquiries.
- Conduct medication order verification and assist with chemotherapy desensitizations as needed.
Qualifications:- Bachelor of Science in Pharmacy
- Illinois Pharmacist License or eligibility to obtain one
- Post-Graduate Year Two (PGY2) Pharmacy Residency in Hematology/Oncology highly preferred
- Board Certification in Oncology Pharmacy (BCOP) preferably within 18 months of hire
- BLS Certification
- Strong knowledge of intravenous admixture, drug therapies, chemotherapy, and cellular therapies
- Ability to collaborate effectively and solve problems independently
