Pharmacy Clinical Supervisor

Job Summary:

The Pharmacy Clinical Supervisor is responsible for the oversight of pharmacy practice programs, medication use systems, and quality initiatives to detect, mitigate, and prevent medication adverse events.  The Pharmacy Clinical Supervisor is responsible for the development and implementation of clinical programs, policies, and procedures ensuring medication use is aligned with evidence-based practice and regulatory standards.

Essential Duties and Responsibilities

• Supports and promotes the mission and values of Covenant Health Ministry.
• Establish and maintain multidisciplinary collaboration with pharmacists, physicians, nursing, and other healthcare professional to optimize patient outcomes.
• Serve on the Pharmacy and Therapeutics Committee to assist with advancement and direction of the medication-use policy, medication safety, and quality initiatives.
• Provide overall formulary management and review all non-formulary medication requests.
• Develop and maintain and ongoing, systematic medication utilization evaluation (MUE) program.
• Oversee program compliance for all applicable Risk Evaluation and Mitigation Strategies at the facility.
• Serve as the Residency Program Director for the PGY-1 Pharmacy Residency Program.
• Maintains direct report responsibility for assigned pharmacy residents and clinical pharmacists, including coaching, competency assessment and clinical practice standards.
• Respond to medication shortages alongside the Pharmacy Inventory Coordinator and the Pharmacy and Therapeutics Committee.
• Developing strategies, goals, programs, and initiatives for advancing patient safety and providing high-quality medication-related patient care.
• Overseeing professional activities of clinical pharmacists and providing guidance and support for advanced clinical integration, standardization, and expansion of pharmacy services to optimize patient outcomes.
• Coordinate in-service education, drug information, competency validation, and other educational services for medical and nursing staff.
• Serves as a pharmacotherapy expert and resource.
• Directs development and implementation of clinical pharmacy services including but not limited to Therapeutic Interchange Program, Renal Dosing, IV to PO interchange Program, Antimicrobial Stewardship, and Anticoagulation Monitor. 
• Manages the Adverse Medication Event Program of the hospital and provides reports to the Medication Safety Committee, Pharmacy and Therapeutics Committee, and Quality Council for review.
• Designs Competency Programs for pharmacists and pharmacy technicians. Provides Director of Pharmacy with documentation of competency assessment for use in annual assessment process
• Provides education and information on pharmaceutical care and drug therapy to hospital clinical staff.
• Participates in community based educational programs as needed.
• Reviews the performance and documentation of clinical activities of all pharmacists.
• Works with the Director of Pharmacy to establish CQI and Performance improvement initiatives and compliance with Board of Pharmacy CQI requirements.
• Completes all competence/skills assessment requirements.
• Provides input regarding personnel actions including but not limited to hiring, transferring, suspending, rewarding, disciplining, or adjusting of grievances involving assigned personnel.  Participates in preparation of performance evaluations. Participates in conflict resolution.
• Participates in multidisciplinary hospital committees as appropriate.
• Participates and coordinates the orientation and training of new staff.
• Other duties as consistent with this role.

• Ability to extend hand(s) and arm(s) in any direction. Ability to pick, pinch, type or otherwise work with fingers rather than the whole hand.  Ability to apply pressure to object with fingers and palm

• Ability to receive detailed information through oral and written communication (e.g., in normal office setting), and to make fine discrimination in sound.
• Ability to plan and perform a wide variety of duties requiring general knowledge of policies and procedures applicable for that area.  Requires considerable judgment to work independently toward general results, devising methods, modifying, or adapting standard procedures to meet different conditions, making decisions based on precedent and policy
• Ability to establish and maintain communications and positive working relationships within the department and with other hospital departments.  Ability to develop relationships with patients, employees, regulatory agencies, vendors, and the public.
• Ability to accept constructive feedback and initiate appropriate actions to correct situations.
• Ability to present clinical findings to a multidisciplinary and make recommendations based on evidence based, patient centered clinical judgement.

Education & Experience

• Current New Hampshire license to practice Pharmacy by the New Hampshire Board of Pharmacy required.
• A Bachelor’s degree in pharmacy from an American Council on Pharmaceutical Education accredited college of pharmacy minimum required; Doctorate preferred.
• 4 years of advanced practice experience required; 5 years of advanced practice strongly preferred.
• Board Certified Pharmacotherapy Specialist (BCPS) or equivalent strongly preferred.
• Completed an ASHP-accredited residency (PGY1 or PGY2) strongly preferred.

Working Conditions/Physical Demands

Must possess the physical and mental abilities to perform the tasks normally associated with this position that involves a combination of sitting, standing, walking, bending, stooping, and reaching. Occasional lifting/carrying up to 25 lbs.  Some stress related to high level of responsibility for quality care.  Visual acuity sufficient for work which deals largely with preparing and analyzing data and figures, computer terminal operation, extensive reading.  Operation of machines (including inspection), using measurement devices, assembly, or fabrication of parts at distances close to the eyes.
