What You Will Do

As the Pharmacy Nurse Care Coordinator, you will provide advanced clinical coordination for patients with bleeding disorders. In this role you will support individualized treatment plans, manage complex patient cases, and collaborates closely with IHI physicians and multidisciplinary teams. The coordinator plays a key role in adherence monitoring, insurance navigation, and specialty-medication logistics.

The Opportunity

  • Instructs pharmacy patients on factor dosing, new product education, assess patient needs and direct patients to immediate care when necessary. Provide detailed patient education on medication administration, bleed recognition, and emergency protocols.
  • Coordinate complex patient cases, facilitate triage and prompt communication of nursing assessments to providers and ensure accurate electronic medical record (EMR) documentation.
  • Provides effective cooperation and communication with other team members in the pharmacy and billing departments to ensure all patient needs are met and financial risks to the IHI are minimized.
  • Works with pharmacists to support individualized dosing plans and factor-utilization reviews.
  • Identifies trends in patient care and needs and make recommendations/implement programs to improve patient services.
  • Monitor adherence, factor usage, and bleed tracking including Infusion log review and education.
  • Conducts home visits and needs assessments.
  • Monitors and facilitates factor order completion and non-factor prescriptions.
  • Assists with home-infusion training and troubleshooting.
  • Assists with scheduling clinic visits, follow-ups, and telehealth appointments.
  • Supports patients in understanding treatment plans and bleed management instructions.
  • Document patient interactions, refill requests, and care-coordination activities in the EMR.
  • Assists the pharmacy team in with collecting clinical data (bleed logs, factor usage, adherence).
  • Maintains accurate patient records and communication logs.

Requirements

  • Valid Rn or LPN license in the state of Indiana
  • ASN or BSN degree
  • All IHI employees are expected to enable multi-factor authentication via their personal smart phone/smart device in order to access IHI systems as a requirement of the role.  

This position is open to residents of the State of Indiana, and requires routine presence at our Center in Indianapolis, IN. Frequency of on-site presence is dependent upon role and department needs.
