Role Description Physical Therapists at Angelcare Home Health Services provide direct, hands-on rehabilitation services to clients in their homes in Fairfield, CA. In this full-time, on-site role, the Physical Therapist evaluates functional mobility, develops individualized treatment plans, and implements evidence-based interventions to improve strength, balance, and endurance. Daily responsibilities include conducting home safety assessments, instructing clients and caregivers in therapeutic exercises and mobility techniques, and documenting progress in accordance with regulatory and agency standards. The role also involves collaborating with an interdisciplinary team, communicating changes in client status, and educating clients about self-management strategies and use of assistive devices. Physical Therapists ensure care is delivered safely, respectfully, and in alignment with each client’s goals and plan of care.

Qualifications

• Active state license as a Physical Therapist (PT) in California and graduation from an accredited Physical Therapy program.

• Clinical skills in assessment, treatment planning, therapeutic exercise, gait training, and functional mobility in home health or similar settings.
• \n
• Knowledge of home health regulations, documentation standards, and familiarity with Medicare, Medi-Cal, and private insurance requirements.
• \n
• Strong communication, education, and interpersonal skills to work effectively with clients, families, caregivers, and interdisciplinary team members.
• \n
• Ability to travel locally to client homes, manage a caseload, and work independently while maintaining professional judgment and safety standards.

• Basic computer proficiency for electronic health records and reporting; bilingual language skills are a plus.
• \n
• Current CPR certification; prior experience in geriatric care, rehabilitation, or home health is preferred.
