    Description

    Avera Health is seeking a compassionate and clinically skilled Physical Therapist to join our Acute Care team within our Healthcare & Medical Services department. In this role, you will provide evidence-based, patient-centered physical therapy services to individuals in the hospital setting, including post-surgical, medical, neurological, and critical care patients. You will perform comprehensive evaluations, develop and implement individualized treatment plans, and collaborate closely with physicians, nurses, case managers, and other members of the interdisciplinary team to optimize patient outcomes and safe discharge planning.

    As an Acute Physical Therapist at Avera Health, you will assess strength, mobility, balance, functional status, and safety, and deliver interventions such as therapeutic exercise, gait and transfer training, bed mobility, balance and neuromuscular re-education, and patient and family education. You will document assessments, goals, progress notes, and discharge recommendations clearly and efficiently in the electronic medical record in accordance with regulatory and payer requirements. You will actively participate in care conferences, recommend appropriate durable medical equipment, and help determine post-acute care needs, such as home health, outpatient therapy, or inpatient rehabilitation.
