Position Overview
Inabia is looking for a Physical Therapist to join our team in a Skilled Nursing Facility (SNF) setting. If you’re passionate about patient-centered care and enjoy collaborating with a supportive team, we’d love to meet you. In this role, you’ll evaluate patients, build individualized plans of care, and deliver skilled interventions that help people move better and live better. You’ll partner closely with the interdisciplinary team and educate patients/caregivers to support safe progress and smooth transitions of care.
Key Responsibilities
Perform evaluations, develop individualized care plans, and deliver therapy services based on patient needs.
Provide daily treatments, track functional progress, and support discharge planning.
Collaborate closely with therapy, nursing, and interdisciplinary teams.
Complete accurate, timely, and compliant clinical documentation.
Participate in skilled nursing rehab programming focused on functional recovery and safe transitions.
Contribute to a team-based approach that prioritizes patient outcomes and quality care.
Qualifications
Active New Mexico Physical Therapist license, or eligibility to practice in the state, required.
Prior SNF experience strongly preferred.
Strong communication, teamwork, and documentation skills.
