    Role Description This full-time, on-site Physical Therapist role is based in Algonquin, IL and serves residents within a senior living community. The Physical Therapist will evaluate residents’ functional abilities, develop individualized treatment plans, and deliver evidence-based therapeutic interventions to improve mobility, strength, balance, and independence. Daily responsibilities include documenting clinical progress, collaborating with community staff and families, and participating in care planning to ensure coordinated, person-centered services. The role also involves educating residents and caregivers on safe mobility, fall prevention, and home exercise programs, as well as contributing to clinical programs and staff training that support overall resident health and wellness.

    Qualifications

    • \n
    • Strong foundation in Physical Therapy and Rehabilitation, with the ability to design and implement individualized treatment plans.
    • \n
    • Demonstrated skills in Patient Care and Home Care, including compassionate, person-centered service and safety-focused practice.
    • \n
    • Understanding of Medicine principles relevant to geriatric and adult rehabilitation, and ability to apply clinical judgment in on-site settings.
    • \n
    • Active state licensure as a Physical Therapist (or eligibility for licensure in Illinois).
    • \n
    • Degree from an accredited Physical Therapy program (DPT or equivalent required).
    • \n
    • Experience working with older adults or in senior living, skilled nursing, or home health environments preferred.
    • \n
    • Effective communication and interpersonal skills, with the ability to collaborate with interdisciplinary teams and educate residents and caregivers.
    • \n
    • Ability to manage a clinical caseload, prioritize tasks, and adapt to changing resident needs within the community setting.
