Offered Rates:

  • PT/OT - $50/hr
  • COTA/PTA - $31-32/hr

Pre-employment documents that you need to email/fax:

  • Up-to-date Resume
  • Valid IDs (Driver License, Passport, Resident Card, Employment Authorization Card)
  • Social Security Card
  • PT/OT License/Registration
  • Two reference letters
  • Physical Health exam results with PPD, MMR lab results
  • Course Completion Certificate/Diploma
  • BLS/CPR Card (optional)
  • Voided Check/Direct deposit form for payroll
