We seeking for a dedicated and compassionate Physical Therapist to join our rehabilitation team. The ideal candidate will provide high-quality patient care in a variety of settings, including hospitals, clinics, and inpatient facilities. This role involves developing individualized care plans, implementing therapeutic interventions, and collaborating with other healthcare professionals to ensure optimal patient outcomes. Experience in pediatrics and geriatrics is preferred.
Responsibilities
Conduct comprehensive evaluations of patients to assess their physical abilities and limitations.
Develop and implement personalized care plans based on individual patient needs and goals.
Administer various physical therapy treatments, including manual therapy techniques and exercise programs.
Monitor patient progress and adjust treatment plans as necessary to achieve desired outcomes.
Educate patients and their families on rehabilitation exercises, home care strategies, and lifestyle modifications.
Collaborate with multidisciplinary teams to ensure cohesive patient care across all settings.
Maintain accurate documentation of patient evaluations, treatment plans, and progress notes in compliance with healthcare regulations.
Participate in discharge planning to facilitate smooth transitions to home and community environment.
Experience
A degree in Physical Therapy from an accredited program is required.
Valid state licensure or eligibility for licensure as a Physical Therapist.
Previous experience in a out-patient clinical setting is preferred but not required.
Knowledge of care plan development and implementation is essential.
Strong communication skills to effectively interact with patients, families, and healthcare team members.
Ability to work collaboratively within a team-oriented environment while maintaining a focus on patient-centered care.
