Role Description This is a full-time, on-site Physical Therapist role based in Bedford, MA. The Physical Therapist will evaluate residents and patients, develop individualized treatment plans, and provide evidence-based therapeutic interventions to improve mobility, functional independence, and quality of life. Daily responsibilities include coordinating rehabilitation services with nursing and other care team members, monitoring progress, adjusting treatment plans, and documenting all assessments and interventions accurately. The role also involves educating residents, families, and caregivers on home exercise programs, fall prevention, and safe mobility techniques, while supporting a compassionate and collaborative care environment.

• Candidates should possess strong skills in Physical Therapy and Rehabilitation, with the ability to design and deliver individualized treatment plans.

• Candidates should possess skills in Patient Care and Home Care, including resident-centered communication and education on safe mobility and home exercise programs.
• \n
• Candidates should possess foundational knowledge of Medicine relevant to geriatrics, orthopedic and neurologic conditions, and post-acute care.
• \n
• Active state license as a Physical Therapist (or eligibility for licensure in MA) and graduation from an accredited Physical Therapy program.
• \n
• Experience working with older adults in rehabilitation, skilled nursing, or similar settings is highly beneficial.

• Strong documentation, time management, and interdisciplinary collaboration skills, with the ability to work respectfully with diverse residents and colleagues.
• \n
• Ability to meet the physical demands of the role, including assisting with transfers and therapeutic exercises in compliance with safety standards.
