Status: PER DIEM

General:

The staff physical therapist provides physical therapy services to patients including evaluations, planning, and individual treatment programs and goals, direct patient treatments, discharge planning and other related duties as required.

Qualifications:

1. Graduate of an approved program of physical therapy.
2. Valid physical therapy license or eligibility for license in the state(s) where services are rendered.
3. Comply with federal and state physical therapy regulations.

Physical, Mental, and Sensory Requirements: Must possess the ability to effectively communicate with therapy staff, patients and others as needed as well as the ability to effectively respond to the environment within the long term care setting. Ability to lift/walk patients, provide active therapy, and stand or sit for long periods of time, all day, if necessary. Ability to read, speak, and understand English. Must be able to lift and transport patients safely. Must be able to support/or recover the balance safely of a patient while assisting in gait training.

Requirements:

Compliance with applicable health laws and regulations, including but not limited to those relating to COVID-19 AND influenza vaccinations.

Must be able to cope with the mental ane emotional stress of the position

Must possess sight/hearing senses or use of prothesis that will enable these senses to function adequately so that the requirements of this position can be fully met

Must be in good general health and demonstrate emotional stability.

Must be able to lift and move residents, equipment, supplies, etc. in excess of Fifty (50) pounds.

Essential Job functions:

The following are essential job duties that must be safely performed with or without reasonable accommodation without posting a direct threat to other employees, patients/residents or self:

1. Upon referral, performs an evaluation to determine the problems and goals of the patient in collaboration with the patient and physician.
2. Develops a treatment program based on initial evaluation to achieve needs and goals and makes appropriate changes in treatment program according to patient’s performance.
3. Administers, schedules, and supervises direct patient care utilizing appropriate physical therapy techniques.
4. Instructs patient and/or patient’s family in home program
5. Completes patient screens based on admission referral quarterly or annually to determine if PT evaluation is warranted.
6. Maintains records pertinent to patient treatment:

1. Evaluation Report
2. Daily treatment record
3. Weekly summary
4. 30 day physician updates
5. Discharge summaries
6. Completion of Medical Information Form
7. Patient care plan
8. Telephone orders.

7. Communicates with patient’s physicians regarding changes or modifications in treatment plan or patient status.
8. Trains new personnel
9. Participates in patient care rounds, rehabilitation team conferences, and conferences with physicians and members of the physical therapy department as necessary regarding progress, problems and specific needs of patient.
10. Actively involved in providing in-service education to all personnel
11. Studies trends and new developments in physical therapy by survey of current literature and attendance at professional meetings and conferences.
12. Conduct department tours for interested individuals/groups
13. Other duties as assigned.
14. Possess math skills.
15. Possess clerical skills.
