A growing Home Health organization is seeking an energetic, compassionate, and driven Physical Therapist (PT) to join its patient-centered care team. Great Compensation Plan!!!

This opportunity is ideal for a Physical Therapist who is passionate about helping patients regain independence, improve mobility, and enhance their quality of life—all from the comfort of their own homes.


Position Responsibilities:


  •  Perform in-home physical therapy evaluations and develop individualized treatment plans 
  •  Provide high-quality therapy services to patients in their home environment 
  •  Focus on improving strength, mobility, balance, safety, and functional independence 
  •  Assess home environments and recommend modifications or equipment to improve patient safety 
  •  Collaborate with physicians, nursing staff, caregivers, and interdisciplinary teams 
  •  Educate patients and families on treatment plans, exercises, and long-term wellness strategies 
  •  Complete timely and accurate documentation in compliance with home health standards 
Qualifications:


  •  Active Physical Therapist license 
  •  Home Health, geriatric, or rehabilitative therapy experience preferred 
  •  Strong clinical assessment and communication skills 
  •  Ability to work independently and manage patient schedules effectively 
  •  Positive, compassionate, and team-oriented attitude 
  •  Passion for patient-centered care and improving lives at home
