Physical Therapist (PT) – Home Health | Williamsburg, Brooklyn, NY | PRN | $72–$85 Per Visit
Stern at Home Therapy is hiring a Physical Therapist (PT) to provide one-on-one care to patients in their homes in and around Williamsburg, Brooklyn, NY.
This opportunity is ideal for a licensed Physical Therapist looking to add flexible PRN visits to an existing schedule without the double-booking, productivity quotas, and clinic burnout common in traditional outpatient settings.
Treat one patient at a time, manage your availability, and provide individualized care with support from an established clinical and administrative team.
What You'll Do:
As a Physical Therapist with Stern at Home Therapy, you will evaluate and treat a primarily geriatric patient population in their homes throughout Williamsburg and the surrounding Brooklyn area.
Responsibilities include:
- Perform physical therapy evaluations and develop individualized plans of care.
- Provide one-on-one treatments based on each patient's functional needs and goals.
- Manage individualized treatment plans with clinical autonomy.
- Help patients improve mobility, strength, balance, safety, and overall function.
- Complete accurate and timely clinical documentation.
- Communicate with patients, families, physicians, and members of the clinical team.
- Coordinate with our administrative team regarding scheduling, patient care, and logistics.
- Build and manage your caseload based on your availability and preferred coverage area.
Compensation:
PRN:
- $72 per 4-unit follow-up visit.
- $85 per evaluation.

Qualifications:
- An active Physical Therapist (PT) license in New York is required.
- Reliable transportation required.
- Strong clinical, communication, and time-management skills.
- Ability to work independently in a home-based setting.
- Home health experience is preferred but not required.
