Oasis Therapy Services is seeking a Physical Therapist to provide rehabilitation services in a skilled nursing facility in Burr Ridge, Illinois.

Responsibilities

• Evaluate residents and develop individualized physical therapy treatment plans.
• Provide treatment focused on strength, balance, gait, transfers, mobility, pain management, and fall prevention.
• Monitor residents’ progress and modify treatment plans as needed.
• Complete discharge planning and provide recommendations for continued care.
• Educate residents, families, and caregivers on exercises, safety, and assistive equipment.
• Supervise Physical Therapist Assistants when assigned.
• Complete accurate Net Health documentation and collaborate with the interdisciplinary care team.

Qualifications

• Active and current Illinois Physical Therapist license.
• Graduate of an accredited Physical Therapy program.
• Current CPR certification.
• Strong communication, organization, and documentation skills.
• Geriatric, skilled nursing, or rehabilitation experience is preferred.
