    Job Title: Physical Therapist

    Salary Range: $45,000 - $48,000 per year

    Job Type: Full-time, 40 hours per week, Monday-Friday with occasional weekend shifts as needed

    Location: Assisted Living Facility (ALF), Independent Living (IL), and Skilled Nursing Facility (SNF) at the same location

    Job Summary:

    We are seeking a highly skilled and compassionate Physical Therapist to join our team at our Assisted Living Facility, Independent Living, and Skilled Nursing Facility at the same location. As a Physical Therapist, you will be responsible for evaluating and treating patients with physical disabilities or injuries to improve their overall mobility and quality of life.

    Key Responsibilities:

    - Conduct comprehensive evaluations of patients' physical abilities and limitations

    - Develop individualized treatment plans based on evaluation findings and patient goals

    - Administer therapeutic exercises and interventions to improve strength, balance, coordination, and range of motion

    - Utilize modalities such as heat, cold, ultrasound, and electrical stimulation to relieve pain and promote healing

    - Educate patients and their families on proper techniques and exercises to continue at home

    - Monitor and document patient progress and adjust treatment plans as needed

    - Collaborate with other healthcare professionals to provide coordinated care for patients

    - Maintain accurate and up-to-date patient records

    - Adhere to all legal and ethical standards in accordance with state and federal regulations

    Qualifications:

    - Bachelor's or Master's degree in Physical Therapy from an accredited program

    - Current state license to practice as a Physical Therapist

    - Minimum of 2 years of experience in a clinical setting, preferably in a senior living or skilled nursing facility

    - Strong knowledge and understanding of physical therapy principles and techniques

    - Excellent communication and interpersonal skills

    - Ability to work independently and as part of a multidisciplinary team

    - Compassionate and patient-centered approach to care

    - Basic computer skills and proficiency in electronic medical records
