    Job Summary

    Nexus at Forest Edge is seeking compassionate and motivated Physical Therapists (PTs) to join our in-house rehabilitation team in Chicago, Illinois. We are hiring for both Full-Time and Per Diem (PRN) positions in our subacute rehabilitation and skilled nursing setting.

    New Full-Time PTs are eligible for a $5,000 Sign-On Bonus! Join a collaborative rehabilitation team dedicated to helping residents maximize their independence, mobility, and quality of life.

    The ideal candidate will evaluate and treat residents with physical impairments, functional limitations, and mobility-related conditions while working closely with an interdisciplinary team focused on exceptional patient outcomes.

    Responsibilities

    • Evaluate residents and develop individualized treatment plans based on functional needs and physician orders
    • Provide skilled physical therapy services in accordance with physician orders and facility policies
    • Implement therapeutic exercise programs, gait training, transfer training, balance activities, and functional mobility interventions
    • Perform ongoing reassessments and modify treatment plans based on patient progress
    • Maintain accurate and timely documentation in compliance with state, federal, and facility guidelines
    • Collaborate with physicians, nursing staff, occupational therapists, speech therapists, and families regarding patient progress and discharge planning
    • Educate patients, caregivers, and staff on mobility techniques, safety awareness, and fall prevention strategies
    • Participate in interdisciplinary care plan meetings
    • Ensure compliance with all regulatory, ethical, and professional standards

    Qualifications

    • Doctorate or Master's Degree in Physical Therapy from an accredited program
    • Current Illinois Physical Therapist license required
    • Experience in skilled nursing, subacute rehabilitation, or geriatric settings preferred
    • Strong clinical reasoning, communication, and interpersonal skills
    • Ability to work independently and collaboratively within a multidisciplinary team
