Serving Counties: Wayne/Seneca OR Ontario/Yates RESPONSIBILITIES: Direct Patient Care Duties: - Develops and revises plan of treatments and discharge plan in conjunction with patient, family, physician, care delivery team and third-party payors including obtaining medical orders and approval of physical therapy services.
- Provides supervision of PTA to ensure patient health, safety, and compliance with plan of care.
- Consults with and educates the patient, family and other team members regarding disease process, self care techniques and preventive strategies. Implements home therapy programs including but not limited to gait training, transfers, ADL's, home modification, care, and use of equipment.
- Cooperatively assists in the orientation of new rehabilitation personnel. Performs other duties and responsibilities as assigned to support the efficient operation of their team, group, and discipline.
- Maintains productivity requirements.
QUALIFICATIONS: - A B.S., M.S. or D.P.T. degree in Physical Therapy
- Possession of a current New York State License in Physical Therapy.
- Ability to physically lift and assist patients in hands on care.
- Minimum of one year experience in Physical Therapy in a hospital setting (not including internship).
- Good verbal, written and interpersaonl communication skills.
Physical Requirements: The position involves frequent sitting, standing, and walking. Occasional bending and pushing may be required. Tasks such as kneeling, squatting, reaching, twisting, balancing, climbing, and pulling are required infrequently. Salary Range - $67,700 - $115,045 The referenced pay range represents the minimum and maximum compensation for this job family. Individual annual salaries/hourly rates will be set within the job’s compensation range, and will be determined by considering factors including, but not limited to, market data, education, experience, qualifications, expertise of the individual, and internal equity considerations. #URM1
