    Apollo Home Healthcare is hiring a full-time or part-time/per diem Physical Therapist (PT) to join our Home Health team, serving patients in Northern California''s Bay Area. In this role, you will provide high-quality physical therapy services to patients in the comfort of their own homes, helping them regain independence and improve their daily functioning. You will oversee a supporting Physical Therapy Assistant and a team of dedicated healthcare professionals who are committed to delivering exceptional patient-centered care. For a limited time, Apollo is offering a $2,500 hiring bonus for a full-time Physical Therapist or a $500 hiring bonus for a part-time/per diem Physical Therapist.

    What You''ll Do

    Provide in-home physical therapy evaluations and treatments

    Assess patients' strength, mobility, balance, and ability to function safely at home

    Create and update individualized plans of care that support independence and recovery

    Help patients regain mobility and confidence in their home environment

    Educate patients and caregivers on exercises, safety, and fall prevention

    Document visits and evaluations in WellSky/Kinnser in a timely manner

    Collaborate with nurses, therapists, and the broader care team to support patient outcomes

    Supervise Physical Therapy Assistants (PTAs) and support their clinical development

    Participate in care planning, case conferences, and discharge planning
