Position Overview
As a Home Health Physical Therapist, you will:
- Provide individualized, one-on-one therapy services in patients’ homes
- Evaluate patients and develop customized treatment plans
- Monitor patient progress and adjust interventions as needed
- Educate patients and caregivers to maximize outcomes
- Collaborate with physicians and interdisciplinary healthcare teams
Qualifications
- Doctorate or Degree in Physical Therapy (DPT or equivalent)
- Minimum 2 years of clinical Physical Therapy experience
- Home Health experience preferred
- Active California Physical Therapist License
- Valid California Driver’s License
- Current CPR certification
- Current TB test and Professional Liability Insurance
- Strong communication, documentation, and organizational skills
- Ability to work independently and manage a flexible caseload
Schedule
- Choose your own hours
- Flexible weekday and weekend opportunities available
- Work location: On the road / patient homes
Compensation & Benefits
