Our mission is to help patients achieve long-term healing and improved quality of life through personalized treatment plans and evidence-based care.Our team includes experienced medical providers, rehab specialists, and support staff who work together to deliver comprehensive care for acute and chronic pain conditions.Responsibilities
Perform patient evaluations and physical therapy assessments
Review medical history, imaging, and physician referrals
Develop individualized treatment plans based on patient needs and goals
Guide patients through therapeutic exercises, modalities, mobility training, and rehabilitation programs
Educate patients on treatment plans, home exercises, posture, stretching, and lifestyle modifications
Monitor patient progress and perform reassessments
Maintain accurate and timely EMR documentation
Collaborate with physicians and multidisciplinary providers to ensure optimal patient outcomes
Requirements
Graduate of an accredited Physical Therapy program
