My client is seeking a Physical Therapist to act as the patient's partner throughout their journey of restoring motion. The Physical Therapist is a key member of the IDT, focusing on care planning and the coordination and delivery of restorative and rehabilitative physical therapy services to participants, working to maximize independence and safety. You'll ensure that each client functions at their personal best, maximizing their quality of life and movement potential across key areas of care; promotion, prevention, diagnosis, treatment, intervention, and rehabilitation.
Conduct Physical Therapy assessments in our facility and occasionally in the home to define participant care needs, preferences, and goals
Actively participate in IDT meetings to develop participant care plans
Deliver and document interventions as agreed upon in participant care plans
Provide active and passive exercise, muscle re-education, gait, functional training of ADLs, and treatments including massage, heat, sound, water, light, or electricity
Supervise Therapy Assistants, providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP)
Provide training and instruction to staff, participants, and caregivers/families on the use and care of therapeutic equipment
Create fitness- and wellness-oriented programs tailored to patients' specific needs
Provide quality, personalized and evidence-based care and proven interventions
Motivate patients during treatment to help them function optimally
Promote clients' healthy lifestyle by improving strength, flexibility, balance, and coordination
Consult and practice with other health professionals
Evaluate effects and monitor and communicate progress
Bachelor's degree in physical therapy; master's or doctoral degree preferred
Two (2) years in clinical setting with frail or elderly population preferred
Valid California Physical Therapy License, or proof of license eligibility
Must obtain CPR and First Aid certification within 180 days of hire and maintain certification thereafter
CPR and First Aid certification upon hire preferred
Physical Therapists that ideally have outpatient, acute, inpatient, or skilled nursing experience.
Ability to manage patients with different types of personalities
Current knowledge of treatment practices
Drive to learn and grow on a constant basis
Professional behavior
Excellent interpersonal communication skills
