Physical Therapist

Job Summary

The role is responsible for coordinating and providing restorative and rehabilitative services to participants at the Center and at home. This involves working in close coordination with the physician, rehabilitation staff, and other interdisciplinary team (IDT) members to maximize participant independence and safety and develop or restore function. The role collaborates with the Occupational Therapist to efficiently operate the rehabilitation department. As a member of the management team, this role facilitates collaborative relationships across the organization and with external stakeholders to promote the organization's values, team culture, and mission.

Duties and Responsibilities

• Conduct assessments to determine care needs, preferences, and goals of the participants and actively participate in IDT meetings to develop participant care plans.
• Deliver and document interventions in accordance with physician orders and/or as agreed upon in the participants' care plans, including but not limited to providing active and passive exercise, muscle re-education, gait, and functional training; treatments such as massage, heat, sound, water, light, or electricity.
• Supervise Therapy Assistants, including providing training, instruction, and guidance needed to implement Functional Maintenance Programs (FMP).
• Provide training and instructions to staff, participants, and caregivers/families in the use and care of therapeutic equipment.
• Order, track, and manage maintenance for equipment.
• Remain alert to pertinent input from other team members, participants, and caregivers and update the IDT promptly of any changes in participants' condition or functional status.
• Work with other members of the IDT to support smooth care transitions between settings (e.g., hospitals, skilled nursing facilities, home, etc.).
• Support enrollment of prospective participants into the program.
• Participate in end-of-life care coordination and support.
• Manage direct reports, including training, supervising, and mentoring.
• Be legally authorized to practice in position location, required.
• Only act within the scope of authority to practice.
• Complete all required documentation in a timely and accurate manner.
• Protect privacy and maintain confidentiality of all procedures and information about team members, participants, and families.
• Follow policies and procedures and participate in any required Quality Improvement activities, staff training, and meetings.
• Communicate regularly with Supervisor and team regarding workload and priorities.
• Timely completion of all mandated trainings and education.
• Timely completion of all mandated occupational health screenings as needed.
• Exercise flexibility in performing assignments as business needs evolve.
• Other duties as assigned.

Required Skills & Experience

• Five years of progressively responsible experience.
• Minimum of two years' experience in a clinical setting with a frail or elderly population.
• Knowledge and skills necessary to treat frail, elderly participants with complex needs.
• Highly motivated, self-directed, able to execute tasks in a quickly changing environment and make sound decisions in emergency situations.
• Two years' supervisory experience with demonstrated ability to mentor and develop team members.
• Ability to process accurate information in a timely manner.
• Excellent leadership, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members.
• Experience leading in a data-driven organization, leveraging reports and data to prioritize and manage people and projects.
• Highly motivated, inspirational leader who is able to innovatively engage, support, and remove barriers for the team to ensure the team consistently meets established goals.
• Excellent clinical, organizational, and communication skills in settings with seniors, their families, and interdisciplinary team members.
• Ability to work independently with minimal supervision.
• Demonstrated ability to prioritize in a fast-paced environment.
• Experience and competency working with people from diverse backgrounds and cultures.

Schedule Notes

5-month assignment, open to temp to perm. M-F 40hrs per week. Candidates must have reliable transportation, as they may be required to escort participants on and off vehicles or visit participants in their homes.
