Job Overview
We are seeking a compassionate and skilled Physical Therapist to join an outpatient and ambulatory care team within a large integrated medical group. This role focuses on evaluating, diagnosing, and treating patients with movement disorders and functional limitations to improve mobility, strength, and overall quality of life.
The Physical Therapist will work collaboratively with physicians and multidisciplinary teams to deliver coordinated, patient-centered rehabilitation services across orthopedic, neurological, and cardiac conditions.
Key Responsibilities
Perform comprehensive patient evaluations and functional assessments
Develop individualized treatment and rehabilitation plans
Provide hands-on therapeutic interventions to improve mobility and function
Monitor, document, and evaluate patient progress and outcomes
Collaborate with physicians, specialists, and care teams
Support program planning, implementation, and evaluation
Educate patients and families on recovery and rehabilitation strategies
Ensure compliance with clinical and regulatory standards
Requirements
Bachelor's Degree in Physical Therapy or related field
Graduate of an accredited Physical Therapy program
Active California Physical Therapist License (or eligibility upon hire)
Basic Life Support (BLS) certification required within 30 days of hire
Experience in outpatient or ambulatory care preferred
