Job Overview
We are seeking a dedicated Physical Therapist to join an interdisciplinary healthcare team providing comprehensive care to frail and elderly participants. The Physical Therapist will coordinate and deliver restorative and rehabilitative therapy services designed to maximize participant independence, mobility, safety, and overall quality of life.
This role offers the opportunity to build meaningful long-term patient relationships while working in a collaborative care environment focused on whole-person care.
Key Responsibilities
- Conduct comprehensive physical therapy evaluations and assessments in clinic and home settings
- Develop individualized treatment plans in collaboration with the interdisciplinary team (IDT)
- Provide therapeutic interventions including exercise, gait training, mobility training, and ADL support
- Deliver treatments utilizing therapeutic modalities such as heat, light, water, massage, sound, and electrical stimulation as appropriate
- Monitor participant progress and modify treatment plans as needed
- Document evaluations, treatments, and participant outcomes accurately and timely
- Supervise Physical Therapist Assistants and support Functional Maintenance Programs (FMP)
- Educate participants, caregivers, and staff on therapeutic exercises and equipment use
- Participate actively in interdisciplinary care planning meetings
Requirements
- Active California Physical Therapist License required
- Bachelor's degree in Physical Therapy required
- Master's or Doctoral degree preferred
- Minimum two (2) years of clinical experience preferred
- Experience working with frail, elderly, or geriatric populations preferred
- CPR and First Aid certification required within 180 days of hire
- Strong clinical assessment, communication, and patient management skills
- Ability to work in both clinic and home-based care settings
