Job Overview

We are seeking a compassionate and skilled Physical Therapist (PT) to join an interdisciplinary healthcare team dedicated to providing comprehensive care for older adults. The Physical Therapist will be responsible for evaluating participants, developing individualized treatment plans, and delivering rehabilitative services that maximize mobility, independence, and safety.

This role offers the opportunity to build meaningful long-term relationships with participants while working in a collaborative, patient-centered environment focused on improving quality of life and functional outcomes.

Key Responsibilities

  • Conduct comprehensive physical therapy assessments in clinical and home-based settings
  • Develop and implement individualized treatment plans based on participant needs and goals
  • Deliver rehabilitative interventions to improve mobility, strength, balance, endurance, and functional independence
  • Provide gait training, muscle re-education, therapeutic exercise, and ADL training
  • Utilize therapeutic modalities as appropriate, including heat, sound, water, light, and electrical treatments
  • Participate actively in interdisciplinary team (IDT) meetings and care planning
  • Monitor participant progress and modify treatment plans as needed
  • Document evaluations, interventions, and outcomes accurately and timely
  • Educate participants, caregivers, and families on therapy goals, home programs, and equipment usage
  • Supervise Therapy Assistants and provide guidance related to Functional Maintenance Programs (FMP)

Requirements

  • Bachelor's Degree in Physical Therapy required
  • Master's or Doctoral Degree in Physical Therapy preferred
  • Active California Physical Therapist (PT) License or proof of license eligibility
  • Minimum two (2) years of clinical experience preferred
  • Experience working with frail, elderly, or geriatric populations preferred
  • CPR and First Aid certification required within 180 days of hire
  • Strong communication, documentation, and patient education skills
  • Ability to work collaboratively within an interdisciplinary healthcare team
